ABSTRACT:
Effective discharges begin with admissions. By implementing technology such as a discharge referral system, hospitals and post-acute care facilities can manage the patient flow process at a holistic, comprehensive level, enabling two key efficiencies:
- Bridge the disparate islands of excellence that exist within the healthcare system, improving coordination across the hospital, and with post-hospital care providers
- Connect the many moving parts of the healthcare delivery process and better manage the complex patient care coordination and discharge planning processes
The time for healthcare organizations to act is now to prevent further delay in discharge planning. By 2030, the U.S. population age 65 and older is projected to exceed 65 million. This increase will undoubtedly strain an already fragile health system. It is imperative that the industry prepare to meet the needs of future patients.

Felicia Wilson brings more than seven years of experience in the healthcare industry to Patient Placement Systems. Previously she was with WellStar Paulding Nursing Center as Assistant Administrator. She has worked as a social worker, admissions director and assistant administrator in nursing homes and hospitals. Wilson holds two licenses in the state of Georgia--Licensed Master Social Worker and Licensed Nursing Home Administrator (LMSW, LNHA). She received her Master of Social Work degree from Florida State University and Bachelor of Social Work degree from Alabama State University. She is currently pursuing her doctoral degree in Health and Human Services from Capella University. Wilson is also an associate professor at Shorter College where she teaches courses in the School of Education and Social Sciences.
